Do Babies Die From Lack Of Attention

Efforts to reduce overall infant mortality rates by enabling women to control the timing of their births through the use of modern contraceptive methods have benefits beyond saving infant lives. Societies with high infant mortality rates also have high fertility rates, in part because couples try to compensate for the infant deaths they have witnessed or experienced. Large families, in turn, reduce the ability of poor parents to invest adequately in the health and education of each child. Thus, a reduction in infant mortality can create an environment in which couples feel less compelled to maintain high fertility levels to ensure the survival of at least some children. As a result, families will eventually grow smaller, and even at existing low income levels, parents will be able to invest more in each child. Given that there are currently relatively few scientifically rigorous studies in this area, there is a clear need for further research about the effects of support interventions directed at parentally bereaved children.

Indeed, there were only 17 studies that met the criteria for this review. All studies, with the exception of one , were comprised of studies about English language interventions that were evaluated in the USA or UK . Furthermore, there is a need for studies with populations sufficiently large enough to make comparisons of the effects for various categories, so that the interventions can be modified to various children's needs. Some studies for example, showed differences in the efficacy of interventions for children at different ages , for girls and boys , for mothers and fathers and for children with different levels of problems at baseline . In the majority of included studies the sample were diverse in ethnicity, but did not analyse effects for different ethnic minority groups. The sample sizes of minority groups were too small to allow the testing of program effects for various groups .

In the studies, the most common deceased parent was the child's father with the remaining caregiver the mother. This is consistent with mortality statistic rates as children under the age of 18 are more likely to experience the death of a father than the death of a mother . The vast majority of newborn deaths take place in low and middle-income countries. It is possible to improve survival and health of newborns and end preventable stillbirths by reaching high coverage of quality antenatal care, skilled care at birth, postnatal care for mother and baby, and care of small and sick newborns. In settings with well-functioning midwife programmes the provision of midwife-led continuity of care can reduce preterm births by up to 24%. MLCC is a model of care in which a midwife or a team of midwives provide care to the same woman throughout her pregnancy, childbirth and the postnatal period, calling upon medical support if necessary.

With the increase in facility births (almost 80% globally), there is a great opportunity for providing essential newborn care and identifying and managing high risk newborns. However, few women and newborns stay in the facility for the recommended 24 hours after birth, which is the most critical time when complications can present. In addition, too many newborns die at home because of early discharge from the hospital, barriers to access and delays in seeking care. The four recommended postnatal care contacts delivered at health facility or through home visits play a key role to reach these newborns and their families.

This integrated analysis reviewed research on breastfeeding and maternal and infant sleep, considering where we are in safeguarding both infant lives and breastfeeding, and how to improve our knowledge and inform policy and practice. The authors found that many breastfeeding mothers and caregivers do sleep with their infants whether intentionally or unintentionally. Introduction and use of the concept of "breastsleeping" is a plausible way to remove the negative connotations of "co-sleeping" and redirect ongoing data-driven discussions and education of best practices of breastfeeding and sleep. As shown in Table 2.1 the leading causes of death differ considerably for children compared to adults, especially elderly adults. For infants, the leading causes of death include congenital anomalies , disorders related to short gestation and low birth weight, and sudden infant death syndrome.

For older children and teenagers, mortality from unintentional and intentional injuries grows in importance. Among adults, as age increases, the relative contribution of injuries decreases, and death rates related to chronic conditions such as heart disease increase sharply. Beginning in adolescence, increasing age also brings increases in causes of death linked to individual behaviors involving diet, exercise, smoking, alcohol use, and similar factors. What this also means is that the share of child deaths from older or younger children also depends on how much progress countries have made on vaccine coverage and other interventions.

There, neonatal deaths account for only 37% of under-5 deaths, whereas in most Asian, European and American countries this share is 50% or higher. This is because many countries across Sub-Saharan Africa still have significant progress to make in the prevention of vaccine-preventable diseases. The included interventions in this review were directed at children from school age up to 18 years of age. This is with the exception of two studies where younger children (0–16) were involved in family therapy sessions .

Most of the studies concerned children who had experienced a parental death from a range of causes, namely illness, accident, suicide or homicide . Commonly parents died because of an illness (65–82%), thereafter due to an accident (15–20%) or suicide/homicide (10–14%). In most studies there was a lack of information about what kind of illness the parent suffered from, where there was information, diseases included those of the heart and cancer . One study compared intervention effects for children who had lost a parent to expected versus unexpected deaths .

One study focused on children during their parent's terminal cancer illness as well as after the parent's death . Finally one study focused on support directed at refugee adolescents who had lost their parents in war . SIDS is the most common cause of death in infants after 1 month of age. It is a diagnosis of exclusion when a postmortem examination, death scene investigation, and review of case records fail to reveal a specific cause of death.

Deaths typically occur between 2 and 4 months of age, and 90 percent of SIDS deaths occur before the child is 6 months old . Environmental factors such as the baby's sleeping position, soft bedding, and cigarette smoke have been implicated as risk factors. An immaturity of the infant's innate ability to control his or her breathing, heartbeat, blood pressure, or arousal level may also contribute to these deaths . Educational programs encouraging parents to put infants to sleep on their backs (the "Back to Sleep" campaign) have been credited as an important factor in the reduction of SIDS rates (Willinger et al., 1998; AAP, 2000b). Supporting families through loss and grief is an integral part of Family-Centred Maternity and Newborn Care.

The emotional impact of perinatal loss is felt by parents, family members, friends and the health care providers caring for the bereaved family. For some parents and families, this is their first major bereavement and they are unfamiliar with the process and effects of grieving. The role of health care services and community organizations is critical in supporting families experiencing loss any time during the perinatal care trajectory. The younger children are especially vulnerable as they are totally dependent on their caregivers. In addition, they often find it more difficult to comprehend what has happened to their deceased parent and what this means .

Consequently, development of supportive interventions and evaluation of bereavement interventions for younger children is an important issue for further research. Involving younger children in evaluations of interventions may require innovative methods, where the children are given the opportunity to express themselves in a way that is adapted to their capacity and cognitive development. Such evaluations may also include qualitative interviews where the children can express themselves in their own words or through creative methods such as art or play .

For example, few of the outcome measures in the included studies concerned children's physical health and somatic symptoms, their situation in school and their peer relationships. Qualitative data from evaluations could also be helpful to identify opportunities to improve current bereavement interventions. Women who practice family planning can avoid high-risk births and therefore reduce their chances of having a baby who will die in infancy. In fact, there is a strong negative correlation between levels of contraceptive use and levels of infant mortality. As would be expected, given the high levels of infant mortality in Sub-Saharan Africa, contraceptive use is much lower there than in other regions; prevalence is below 10% is 16 countries and 5% or less in 10 .

Table 2.4, which shows trends in infant, fetal, and perinatal mortality rates since 1950, reveals continuing mortality decreases in the last half-century. In 1999, the infant mortality rate in the United States reached a low of 7.1 infant deaths per 1,000 live births, or 28,371 total infant deaths. After infancy, the mortality rate drops significantly and does not rise again to similar rates until people reach their mid-50s. This chapter summarizes information about the death rates and leading causes of death for children of different ages. The discussion begins, however, by briefly putting life-threatening illness and death during childhood in the broader context of child health and illness in the United States. The chapter ends with a short discussion of conclusions and implications for health care providers and policymakers.

Big countries like Brazil and China reduced their child mortality rates 10-fold over the last 4 decades. Other countries – especially in Africa – still have high child mortality rates, but it's not true that these countries are not making progress. In Sub-Saharan Africa, child mortality has been continuously falling for the last 50 years (1 in 4 children died in the early 60s – today it is less than 1 in 10).

Over the last decade this improvement has been happening faster than ever before. Rising prosperity, rising education and the spread of health care around the globe are the major drivers of this progress. At the same time, support also needs to be directed at the children. In the evaluation of a parental guidance program, the remaining parents expressed that they perceived a need for more support directed to their children .

In one of the included studies, both children and parents indicated that they wished to discuss grief-related experiences with other people who had similar experiences . Being and connecting with other bereaved children can be helpful for children who attend a support group, as it can help them to feel less isolated and alone . Some children avoid talking about their problems or showing their feelings as they try to protect their remaining parent or other people around them.

Can Babies Die From Lack Of Attention This can sometimes be misinterpreted as a sign that the child is not affected by the loss . The included effect-evaluated interventions were not sufficient for all children. Studies indicated that some children may need more intensive support or additional support . Therefore, it is important to reassess children's further needs for support at the end of an intervention .

The overall results suggest that even relatively brief supportive interventions can prevent children from developing more severe problems after the loss of a parent . The randomized controlled studies of "The Family Bereavement Program" stand out among the included studies, as the intervention has been evaluated several times, with different outcomes and longitudinally (6 year follow-up period) . After the first included effect study that was published in 1992, the support program has been subsequently revised and refined. The program consists of a total of 14 sessions, including separate groups for caregivers, children and adolescents; joint activities for children and their caregivers; and individual family meetings . The studies concerning "The Family Bereavement Program" from the year 2003 and onwards concern the same version of the support program whose effects have been evaluated from different perspectives. The evaluations of the program also include fidelity of program implementation, assessed as attendance and implementation of the items described in the manuals .

The results showed positive effects for both children and caregivers. Studies of the program indicated that some children and families may require more intensive interventions or additional support as the intervention itself is brief. These improvements are also essential to increase survival rates among babies born to poor women worldwide.

The chances of significant gains in infant survival are greatly enhanced when broad-based strategies are combined with expanded access to family planning services. For children who died of cancer in 1997, about 58 percent of deaths occurred in hospital inpatient units, about 36 percent occurred at home, and 2.8 percent occurred in hospital outpatient settings . Of those who died in the hospital, nearly half died in the pediatric intensive care unit and an additional third in the oncology ward. Regional variations in medical practice, health care resources, urban or rural place of residence, and other factors could account for the different pattern in the national data.

Two hundred years ago the child mortality rate was extremely high around the world –more than 40% of all children died. Since then the child mortality rate has declined more than 10-fold. Because we need to further reduce child deaths we are studying the causes of death of children today and how it was possible to improve child health so very substantially in the past in ourentry on child mortality. Perinatal loss and grief is a one-of-a-kind experience, unique to each person. It includes physical, emotional, intellectual, spiritual, social and occupational elements.

High quality family-centred bereavement care includes compassion, validation of the loss, open communication, informed choice and individualized care. Compassionate communication, the most important element of bereavement care, is required during all aspects of care for parents throughout and following loss. Effective care and support is always tailored to meet the individual needs and preferences of bereaved parents – and takes into account their religious and cultural practices – without making assumptions about their wishes and decisions. Health care services and organizations have a critical role in supporting the wellbeing of families experiencing loss at any time during the perinatal care trajectory. A family that experiences a perinatal loss needs access to support and referrals in the community.

Because a perinatal loss usually occurs in a health care facility, the parents may return home without the necessary support. It is up to hospital and community HCPs to ensure that families receive follow-up and appropriate support services — all with their informed consent. It is not uncommon to feel anger toward others in society, especially friends and family members who do not acknowledge the child, or who discount parental grief responses and go on as if the child's death did not happen. Couples who experience a miscarriage may experience their grief all alone, as few people may have known of the pregnancy loss.

Today, those absolute mortality numbers are only 7.4 and 3.8 respectively, but there is still nearly a two-fold difference in mortality rate. Absolute inequality has decreased dramatically, but the ratio between the two – a measure of relative inequality – has only decreased a little. Our concern will then turn towards countries where the chances of child mortality is high, and the number of births increasing. Countries where children are most likely to die – Somalia, Chad, Central African Republic, Sierra Leone, Nigeria, and Mali – will all have an increasing number of children in the coming decades.

Progress on reducing child deaths will here therefore become a race between declining child mortality rates and an increasing number of children. Of course, the death of every child is an enormous tragedy, and in many countries far too many children die because of causes we know how to prevent and treat. As the map here shows, today the highest child mortality rates are in Sub-Saharan Africa, where we still have countries with child mortality rates greater than 10% — this means that one out of 10 children born never reach their 5th birthday. Communities and decision makers need to be informed that neonatal deaths are a huge portion of child deaths, and need therefore to receive adequate attention.

Improved registration and increasing the availability and use of relevant information in programmes and to decision makers is essential if health care for newborn babies and their mothers is to be given adequate attention. This term played an important role by focusing attention on a major category of postneonatal infant death, providing support to grieving families, and diminishing the guilt and blame characteristic of these deaths. Unfortunately, the application of this term has become increasingly controversial. According to the definition proposed in 1969, despite slight changes suggested in 1989, SIDS remains a diagnosis of exclusion. Although this syndrome has several distinctive features, including age distribution and apparent occurrence during sleep, there has been reluctance to include these features in the definition.

The problems created by the lack of an adequate definition are discussed. Some health care facilities provide annual memorial services or a burial area to allow group commemoration of pregnancy losses and infant deaths. Bereaved families and HCPs can participate in services or events, which may be held at the health care facility, a local cemetery or elsewhere. The death of a parent is a highly stressful life event for bereaved children.

Several studies have shown an increased risk of mental ill-health and psychosocial problems among affected children. The aims of this study were to systematically review studies about effective support interventions for parentally bereaved children and to identify gaps in the research. When a child dies, whether a baby, a child, or an adult child, there may also be grandparents who also grieve. Grandparents experience the same feelings of grief as parents, albeit the loss is a different experience. It is particularly difficult for grandparents to experience a grandchild's death when they feel their lives have been lived fully but the child's life was severed prematurely. Especially when an adult child has died, the grandparents' feelings of loss and grief are often not acknowledged.

Too, some elderly adults do not easily share their feelings with others, even their own children. Some sudden and unexpected deaths can be explained by the post-mortem examination . The examination could show, for example, an infection or other illness such as a genetic disorder that could have caused the death.
